By the time you have read this, you should hopefully know a little about us, whether through our website or through the people who have referred you.

We expect your experiences in our office will be enjoyable, uplifting, and rewarding. For this reason, please do not hesitate to ask us any questions and we will endeavour to assist you in every way possible.

For your first visit, we ask you to be at our office ten to fifteen minutes prior to your appointment time. Please note our location so you can find us easily. During this time, we will ask you to fill out our health questionnaire (if you haven’t brought your downloaded one with you).

To save time, please complete an  Adult Intake Form or Paediatric Intake Form.
